Document request fees are NON-REFUNDABLE as per policy. Please ensure this form is what you require before submitting a request for this document.
Document requests can only be submitted by an NSNMR applicant or registrant. We will not process requests from third-party organizations. If you are the receiving organization of this document, please kindly direct the applicant or registrant to contact registration@nsnmr.ca to make the request.
Fees are in Canadian funds, includes the Harmonized Sales Tax (HST), and must accompany your request. We will complete this as soon as possible once we receive both the request and payment. Please note: you have 14 days to pay the invoice for your document request. If the invoice is not paid within 14 days, the document request will be withdrawn.
